thy_o life_n may_v be_v present_a with_o thou_o and_o assist_v thou_o who_o office_n shall_v be_v principal_o about_o three_o thing_n the_o first_o be_v that_o have_v commit_v the_o charge_n of_o thy_o soul_n unto_o such_o in_o this_o extremity_n they_o will_v make_v thou_o some_o spiritual_a exhortation_n comfort_v and_o persuade_v thou_o to_o patience_n and_o constancy_n to_o the_o end_n thou_o may_v have_v the_o great_a reward_n in_o heaven_n for_o conform_v thyself_o to_o god_n his_o holy_a will_n &_o pleasure_n in_o this_o and_o every_o other_o affliction_n also_o they_o shall_v exhort_v thou_o that_o all_o care_n and_o all_o love_n of_o transitory_a thing_n of_o this_o world_n set_v a_o side_n with_o all_o thy_o hart_n thou_o be_v attentyve_a to_o the_o health_n of_o thy_o soul_n thank_v our_o lord_n god_n continual_o for_o his_o innumerable_a benefit_n bestow_v upon_o thou_o second_o that_o while_o they_o assist_v thou_o they_o do_v make_v thou_o many_o good_a and_o profitable_a demand_n first_o they_o ought_v to_o demand_v if_o thou_o firm_o believe_v all_o the_o article_n of_o the_o christian_n faith_n and_o that_o our_o holy_a mother_n the_o church_n believe_v and_o hold_v moreover_o they_o ought_v to_o ask_v thou_o if_o thou_o have_v grief_n and_o sorrow_n for_o all_o thy_o offence_n commit_v against_o god_n ask_v pardon_n with_o a_o humble_a and_o contrite_a hart_n and_o if_o thou_o purpose_v never_o to_o offend_v he_o more_o &_o if_o for_o his_o love_n thou_o willing_o pardone_v all_o those_o that_o have_v offend_v thou_o and_o they_o shall_v also_o put_v thou_o in_o remembrance_n that_o if_o thou_o have_v take_v away_o the_o good_a name_n or_o the_o good_n of_o any_o person_n that_o thou_o actualy_a if_o it_o be_v in_o thy_o power_n do_v without_o delay_n restore_v it_o and_o after_o they_o shall_v exhort_v thou_o to_o put_v all_o thy_o hope_n and_o set_v all_o thy_o love_n in_o our_o most_o mild_a and_o love_a redeemer_n beseech_v he_o with_o fervent_a desire_n that_o he_o will_v give_v thou_o perfect_a faith_n hope_n and_o charity_n and_o that_o for_o the_o merit_n of_o his_o holy_a passion_n and_o for_o his_o infinite_a mercy_n he_o will_v give_v thou_o the_o everlasting_a glory_n of_o heaven_n and_o if_o the_o sick-man_n shall_v not_o to_o these_o demand_n answer_v like_o a_o good_a christian_n then_o his_o choose_a friend_n before_o mention_v shall_v with_o all_o diligence_n do_v their_o endeavour_n to_o remove_v from_o he_o every_o ill_a apprehension_n and_o disposition_n of_o spirit_n persuade_v he_o with_o love_v &_o sweet_a word_n how_o much_o it_o be_v needful_a at_o this_o time_n to_o prepare_v himself_o to_o receyve_v devout_o all_o the_o sacrament_n of_o the_o church_n and_o whole_o and_o thorough_o to_o do_v according_a to_o the_o demand_n and_o question_n above_o specify_v to_o the_o office_n of_o this_o faithful_a and_o devout_a friend_n or_o friend_n it_o shall_v also_o belong_v to_o provyde_v that_o the_o sick_a do_v not_o die_v without_o the_o sacrament_n and_o that_o the_o image_n of_o the_o crucifix_n and_o of_o
